    An overview of technologies and devices against COVID-19 pandemic diffusion: virus detection and monitoring solutions

    none5siThe year 2020 will remain in the history for the diffusion of the COVID-19 virus, originating a pandemic on a world scale with over a million deaths. From the onset of the pandemic, the scientific community has made numerous efforts to design systems to detect the infected subjects in ever-faster times, allowing both to intervene on them, to avoid dangerous complications, and to contain the pandemic spreading. In this paper, we present an overview of different innovative technologies and devices fielded against the SARS-CoV-2 virus. The various technologies applicable to the rapid and reliable detection of the COVID-19 virus have been explored. Specifically, several magnetic, electrochemical, and plasmonic biosensors have been proposed in the scientific literature, as an alternative to nucleic acid-based real-time reverse transcription Polymerase Chain Reaction (PCR) (RT-qPCR) assays, overcoming the limitations featuring this typology of tests (the need for expensive instruments and reagents, as well as of specialized staff, and their reliability). Furthermore, we investigated the IoT solutions and devices, reported on the market and in the scientific literature, to contain the pandemic spreading, by avoiding the contagion, acquiring the parameters of suspected users, and monitoring them during the quarantine period.openR. de Fazio, A.     Precision oncology for RET-related tumors

    Aberrant activation of the RET proto-oncogene is implicated in a plethora of cancers. RET gain-of-function point mutations are driver events in multiple endocrine neoplasia 2 (MEN2) syndrome and in sporadic medullary thyroid cancer, while RET rearrangements are driver events in several non-medullary thyroid cancers. Drugs able to inhibit RET have been used to treat RET-mutated cancers. Multikinase inhibitors were initially used, though they showed modest efficacy and significant toxicity. However, new RET selective inhibitors, such as selpercatinib and pralsetinib, have recently been tested and have shown good efficacy and tolerability, even if no direct comparison is yet available between multikinase and selective inhibitors. The advent of high-throughput technology has identified cancers with rare RET alterations beyond point mutations and fusions, including RET deletions, raising questions about whether these alterations have a functional effect and can be targeted by RET inhibitors. In this mini review, we focus on tumors with RET deletions, including deletions/insertions (indels), and their response to RET inhibitors

    Evaluation of polygenic determinants of non-alcoholic fatty liver disease (NAFLD) by a candidate genes resequencing strategy

    NAFLD is a polygenic condition but the individual and cumulative contribution of identified genes remains to be established. To get additional insight into the genetic architecture of NAFLD, GWAS-identified GCKR, PPP1R3B, NCAN, LYPLAL1 and TM6SF2 genes were resequenced by next generation sequencing in a cohort of 218 NAFLD subjects and 227 controls, where PNPLA3 rs738409 and MBOAT7 rs641738 genotypes were also obtained. A total of 168 sequence variants were detected and 47 were annotated as functional. When all functional variants within each gene were considered, only those in TM6SF2 accumulate in NAFLD subjects compared to controls (P = 0.04). Among individual variants, rs1260326 in GCKR and rs641738 in MBOAT7 (recessive), rs58542926 in TM6SF2 and rs738409 in PNPLA3 (dominant) emerged as associated to NAFLD, with PNPLA3 rs738409 being the strongest predictor (OR 3.12, 95% CI, 1.8-5.5, P 0.28 was associated with a 3-fold increased risk of NAFLD. Interestingly, rs61756425 in PPP1R3B and rs641738 in MBOAT7 genes were predictors of NAFLD severity. Overall, TM6SF2, GCKR, PNPLA3 and MBOAT7 were confirmed to be associated with NAFLD and a score based on these genes was highly predictive of this condition. In addition, PPP1R3B and MBOAT7 might influence NAFLD severity

    The outbreak of COVID-19 pandemic has acted as a significant stress test for healthcare systems worldwide, due to the need for hospitalization of an increasing number of infected patients. The shift of massive resources to the acute needs of the pandemic led to an upheaval of the usual diagnostic and therapeutic pathways of chronic diseases, including thyroid cancer disease. The motto was to reduce crowding at clinics and to maintain essential health services. However, thyroid cancer clinical practice recommendations already encouraged physicians to reduce “low-value” care: in particular, to avoid screening of general population, to reduce the number of unnecessary biopsies, and to adopt a conservative approach to indeterminate thyroid nodules and low-risk thyroid cancer

    Lung adenocarcinoma is the tumor with the highest number of switch genes (298) compared to its normal tissue, followed by thyroid (227) and colorectal (183) cancers. Switch genes codifying for kinases were 14,7 and 3 respectively.We looked for three homology sequences identified across vemurafenib targets and we found that thyroid cancer and lung adenocarcinoma have a similar number of putative targetable switch genes kinase (5-6); on the contrary, colorectal cancer has just one,with minor homology sequence

    Cetuximab continuation after first progression in metastatic colorectal cancer (CAPRI-GOIM): A randomized phase II trial of FOLFOX plus cetuximab versus FOLFOX

    Background: Cetuximab plus chemotherapy is a first-line treatment option in metastatic KRAS and NRAS wild-type colorectal cancer (CRC) patients. No data are currently available on continuing anti-epidermal growth factor receptor (EGFR) therapy beyond progression. Patients and methods: We did this open-label, 1:1 randomized phase II trial at 25 hospitals in Italy to evaluate the efficacy of cetuximab plus 5-fluorouracil, folinic acid and oxaliplatin (FOLFOX) as second-line treatment of KRAS exon 2 wild-type metastatic CRC patients treated in first line with 5-fluorouracil, folinic acid and irinotecan (FOLFIRI) plus cetuximab. Patients received FOLFOX plus cetuximab (arm A) or FOLFOX (arm B). Primary end point was progressionfree survival (PFS). Tumour tissues were assessed by next-generation sequencing (NGS). This report is the final analysis. Results: Between 1 February 2010 and 28 September 2014, 153 patients were randomized (74 in arm A and 79 in arm B). Median PFS was 6.4 [95% confidence interval (CI) 4.7-8.0] versus 4.5 months (95% CI 3.3-5.7); [hazard ratio (HR), 0.81; 95% CI 0.58-1.12; P = 0.19], respectively. NGS was performed in 117/153 (76.5%) cases; 66/117 patients (34 in arm A and 32 in arm B) had KRAS, NRAS, BRAF and PIK3CA wild-type tumours. For these patients, PFS was longer in the FOLFOX plus cetuximab arm [median 6.9 (95% CI 5.5-8.2) versus 5.3 months (95% CI 3.7-6.9); HR, 0.56 (95% CI 0.33-0.94); P = 0.025]. There was a trend in better overall survival: median 23.7 [(95% CI 19.4-28.0) versus 19.8 months (95% CI 14.9-24.7); HR, 0.57 (95% CI 0.32-1.02); P = 0.056]. Conclusions: Continuing cetuximab treatment in combination with chemotherapy is of potential therapeutic efficacy in molecularly selected patients and should be validated in randomized phase III trials

    Stability analysis of a three-wheeled motorcycle

    In this work the modal analysis of a three-wheeled tilting motorcycle is presented. This new kind of vehicle has two front wheels and a single rear wheel, but is driven like a common motorcycle. In order to study the stability of the system in straight running, two models have been developed: a simplified motorcycle model, with locked suspensions and rigid and thin tires and a more accurate model having 14 degrees of freedom, in which the stiffness and damping of suspensions and the radial stiffness of tires have been taken into account. In both models the frame has been considered as rigid and the driver was assumed to be fixed to the frame. A linear model with transient behaviour has been employed for describing the tire behaviour. A reference model of a two wheeler with similar inertial properties has been also developed for comparison

    RSVP is an Internet protocol designed to reserve network resources. Availability of optimized free implementations of RSVP daemon and the development of extensions designed to improve RSVP's behaviour have increased the attention towards the protocol. Though RSVP has been designed to fulfill QoS needs of multimedia applications, development of compliant applications is slowed by complexity of logic behind the access to QoS. This paper describes the current problems related to the deployment of RSVP into multimedia applications and the QOSMIC architecture in support to multimedia applications that tries to solve these problems. The QOSMIC elements allow a user to reserve resources without requiring the update of multimedia applications neither on the client side nor on the server one, allow a receiving user to require RSVP Path messages needed to start reservations and allow a centralized management of QoS. A test implementation of QOSMIC is also briefly described

    The analysis of the steady-state cornering behaviour of a vehicle is generally based on the classical single track model. As a consequence, some results, such as the handling diagram and the understeer gradient, are not general and should be used only for the category of vehicles that they represent. A general approach, which is not dependent on the vehicle model, is presented in this paper. It is shown how the handling performances of any vehicle can be investigated by the properties of some functions of two variables, called handling surfaces.
